1. MEMBERSHIP REGISTRATION PROCEDURE (TO BECOME KSPF MEMBER )
NOTE: MEMBERSHIP REGISTRATION PROCEDURE IS DIFFERENT FOR PHYSIOTHERAPIST WHO STUDIED IN THE COLLEGES/UNIVERSITIES WITHIN KARNATAKA AND FOR PHYSIOTHERAPIST WHO STUDIED IN THE COLLEGES/UNIVERSITIES OUTSIDE KARNATAKA WITHIN INDIA.
TWO DIFFERENT MEMEBRSHIP REGISTRATION PROCEDURE.
A. Eligibility to become Member of KSPF (Applicable only for Physiotherapist who studied in college/ institution affiliated to University within the Karnataka State )
- Physiotherapist who persuded Diploma in Physiotherapy or Bachelor in Physiotherapy or Master in Physiotherapy course from institutions recognized by the state Government of Karnataka or institutions recognized by any other State Government within Karnataka shall be eligible to become the Life Member of KSPF.
- Membership procedure is limited for certain Physiotherapy courses (Physiotherapy courses from Distance Education Universities or Distance Education courses are not eligible for KSPF membership). As per UGC, distance education/ correspondence course ( View letter) in Physiotherapy degree is not a vaild course. Please contact KSPF for more details.
- The enrolled members who paid fee Rs.1000/- will be issued Life Membership Registration Number, KSPF certificate with life time validity. Certificate will be issued only for Physiotherapist who become memebers w.e.f 1st April 2015 onwards. Members who enrolled before 1st April 2015 by paying Rs.550/- shall apply for certificate with additional charges.
- Type of Membership: Life Membership
-Memebrship fee: Rs.1000/-
-The enrolled members who paid fee Rs.1000/- will be issued Life Membership Registration Number, KSPF certificate with life time validity.
Procedure:
To obtain KSPF membership registration, send filled application form with document directly to KSPF Office

The hard copy of Membership registration form has to be filled completely by the applicant. (

Affix passport size photo in the space left for photo in the form and enclose three passport size photos, the documents such as photocopy of karnataka resident address proof only (Passport/ Ration Card/ Driving license / Voter id/ any address proof supporting documents) and self attested photocopy of Diploma or Degree Certificates. All the enclosing documents must be attested/ Self attested.

Total fee of Rs.1000/-( one thousand rupees only) towards admission fee of Rs.100/-, Membership fee of Rs.650/-, and Certificate fee Rs.250/-) by Demand Draft payable at Bangalore favoring "Karnataka State Physiotherapy Federation" shall be enclosed.(Certificate will be issued only for Physiotherapist who become memebers w.e.f 1st April 2015 onwards. Members who enrolled before 1st April 2015 by paying Rs.550/- shall apply for certificate with additional charges Rs.250/-)
KSPF MEMBERSHIP CERTIFICATE APPLICATION FORM : For member who beccame member of Karnataka State Physiotherapy Federation before 1st April 2015

You can send the "Physio Archive Enrollment form" along with membership registration form with documents to KSPF to enroll your consultation details into KSPF "Physio Archive" for patient referral.

Send the filled application form with supporting documents by post to the address mentioned in the bottom of the application form.
- Once the Application form recieved by KSPF, the verfication process starts and takes 12 to 24 weeks. Once verfication complets and eligibility criteria fulfilled, you will recieve an email with your KSPF membership registration number.
- After a period of time your KSPF certificate will be sent to you and same will be intimated.
Privileges of Members:
- To be present speak and vote at any meeting of the members of the KSPF on any matter which may be submitted for its co-ordination.
- All members of the KSPF shall be entitled to receive official communication from Karnataka State Physiotherapy Federation.
- Job assistance by regular updates on Job opportunities in Karnataka to all the members through Karnataka State Physiotherapy Federation.
- To attend and participate in Continuous Physiotherapy Education programs organized by KSPF in regional and state level.
- To participate in publishing Literature, Magazines, Journals, Newsletters etc. for the uplift of Physiotherapy through Karnataka State Physiotherapy Federation.
- To convene meetings, organize camps, lecture, seminars, publish literature, to educate general public about physiotherapy.
- To provide free Physiotherapy treatment to Rural community, poor, downtrodden and needy at village level by conducting Physio Camps periodically
- Participate in the programs to create awareness and spread physical health amongst Rural and Urban women, children and men and to educate them for prevention and health care benefits.
- To form regulatory body (Karnataka state Council for Physiotherapy) that will secure Physiotherapist who are a member of Karnataka State Physiotherapy Federation.
- To interact with Online Discussion about Physiotherapy.
- Participate in Scientific and Research Development in Physiotherapy by presenting Papers, Case studies, Articles etc through Karnataka State Physiotherapy Federation.
- Free enrollment of KSPF Member’s Physiotherapy Rehabilitation Centre or Clinic’s details to display in Karnataka State Physiotherapy Federation website.
B. Eligibility to become Member of KSPF (Applicable for Physiotherapist who studied in the Universities outside Karnataka State within India)
Physiotherapist who persuded Diploma in Physiotherapy or Bachelor in Physiotherapy or Master in Physiotherapy course from Universities outside Karnataka recognized by any other State Government within India shall contact General Secretary, KSPF, before applying for KSPF Membership registration to check for elgibility.
Procedure to apply for Eligibility:
Physiotherapist studied from Universities outside Karnataka within India shall apply for eligibility through email.

2. ENROLLMENT TO "PHYSIO ARCHIVE"
( TO ENROLL YOUR CLINICAL PRACTICE INTO PATIENT REFFERAL SYSTEM)
PROCEDURE:
A Member of KSPF can enroll their Physiotherapy practice/Consultation details to "Physio Archive" of KSPF by sending filled application form directly to KSPF Office.
If you are not a member of KSPF, you can send the "Physio Archive" Enrollment form along with membership registration form with documents to KSPF.
THE CONSULTATION DETAILS OF APPLICANT WHO SEND ''PHYSIO ARCHIVE'' FORM WILL BE DISPLAYED IN PHYSIO ARCHIVE'OF KSPF.
ENROLLING TO PHYSIOARCHIVE WILL BE BENEFITED TO PATIENT FROM VARIOUS PLACES IN KARNATAKA AS MANY PATIENTS ARE CALLING FROM DIFFERENT PLACES IN KARNATAKA TO CONSULT PHYSIOTHERAPIST AT THEIR PLACE.

GOOD STANDING CERTIFICATE APPLICATION FORM
Eligbility for " Good Standing Letter" : Applicant shall be a member of KSPF minimum 6 months before applying for Good Standing letter.
Total fee of Rs.1050/-( one thousand rupees fifty only) towards 'Good standing Certificate' shall be paid by Demand Draft payable at Bangalore favoring "Karnataka State Physiotherapy Federation" shall be enclosed with a requesting letter and the certificate will be posted with karnataka Satate. (Additional charges shall be bared by the applicant towards postal/courier charges)
