A PHYSIOTHERAPY CLINIC/CENTRE IN KARNATAKA STATE SHALL REGISTER UNDER " THE KARNATAKA PRIVATE MEDICAL ESTABLISHMENT ACT 2007 & 2009" THE KARNATAKA PRIVATE MEDICAL ESTABLISHMENTS ACT, 2007 THE KARNATAKA PRIVATE MEDICAL ESTABLISHMENTS ACT, 2007
Procedure: 1. Establish your Physiotherapy Centre as per the Minimum Standard for Physiotherapy Centre 2. Apply and arrange for following documents: 1. Passport size Photo of the owner. 2. Council registration Certificate or Association Membership Certificate within Karnataka. In Karnataka the Association is 'Karnataka State Physiotherapy Federation'. 2. Occupancy Certificate from Town Planning Section, Head Office, BBMP. or Muncipal Office in case of other than Bangalore. Note: Usuall this will be available with the Owners of the Property 3. Biomedical waste mangment certificate 4. Fire Safety Certificate. 5. Trade licence certificate from BBMP or Muncipal Office 6. Pollution control Board permission certificate. 7.Floor Plans. 8. Front View photo of the Physiotherapy Centre.

Job type: Consultant Registration number: KSPF membership registration number (Not an Option) Certificate: Upload KSPF certificate or Degree Certificate if accepted by KPME.

STEP:11- Concerened officer will verify your orginal documents and inspect your centre or clinic Step:12- All the requirements meet office may approve and issue the registration certificate.
